Background of the study

Toxic epidermal necrolysis (TEN) is a rare, but severe, life-threatening dermatologic condition characterized by widespread epidermal detachment and mucosal involvement, often triggered by drug reactions (Hussein et al., 2023). The condition is associated with high morbidity and mortality rates, requiring immediate medical intervention to manage fluid balance, prevent secondary infections, and provide pain relief (Ebrahim et al., 2024). Nurses play a critical role in the early identification and management of TEN, as well as in the coordination of care with dermatologists and other specialists. Given the complexity of TEN, early diagnosis and prompt treatment can significantly improve the patient's prognosis (Weiss & Chang, 2024).

At Ondo State Specialist Hospital, nurses are tasked with managing patients with various dermatological conditions, including TEN. However, due to the rarity of the condition, there may be limited exposure to it in clinical practice, leading to gaps in knowledge and potential delays in treatment (Khatri et al., 2023). The lack of standardized protocols for the management of TEN in many healthcare settings further complicates effective care delivery. This study seeks to assess nurses' knowledge and management practices related to TEN at Ondo State Specialist Hospital to identify areas for improvement in care delivery.

Statement of the problem

Toxic epidermal necrolysis, although rare, is a serious condition that demands immediate intervention. Nurses at Ondo State Specialist Hospital may encounter challenges in managing TEN due to limited exposure or knowledge gaps about the condition. Given the severity of the disease, inadequate knowledge and delayed management could result in increased patient morbidity and mortality. This study aims to assess the level of knowledge and management strategies among nurses for managing toxic epidermal necrolysis at Ondo State Specialist Hospital.

Objectives of the study

  1. To assess the level of knowledge of nurses regarding the recognition and management of toxic epidermal necrolysis at Ondo State Specialist Hospital.

  2. To evaluate the management strategies employed by nurses in treating patients with toxic epidermal necrolysis.

  3. To identify the challenges nurses face in managing patients with toxic epidermal necrolysis at Ondo State Specialist Hospital.

Research questions

  1. What is the level of knowledge of nurses regarding toxic epidermal necrolysis at Ondo State Specialist Hospital?

  2. How do nurses manage patients diagnosed with toxic epidermal necrolysis at Ondo State Specialist Hospital?

  3. What challenges do nurses face in managing toxic epidermal necrolysis at Ondo State Specialist Hospital?

Research hypotheses

  1. Nurses at Ondo State Specialist Hospital have inadequate knowledge regarding the recognition and management of toxic epidermal necrolysis.

  2. There is a significant relationship between nurses' knowledge of TEN and their management practices.

  3. Nurses at Ondo State Specialist Hospital face significant challenges in managing patients with toxic epidermal necrolysis due to lack of exposure or training.

Scope and limitations of the study

This study will focus on nurses working in the dermatology and intensive care units of Ondo State Specialist Hospital. The scope will be limited to assessing their knowledge and management practices regarding toxic epidermal necrolysis. One limitation is that the condition is rare, which may limit the sample size of affected patients. Additionally, the findings may not be generalizable to hospitals outside the region.

Definitions of terms

  • Toxic epidermal necrolysis (TEN): A life-threatening condition characterized by widespread detachment of the skin and mucous membranes, often triggered by drug reactions.

  • Management strategies: The interventions used by nurses to manage TEN patients, including pain control, fluid resuscitation, and prevention of secondary infections.

  • Knowledge: The understanding of the clinical signs, pathophysiology, and treatment options for toxic epidermal necrolysis.
